Check In Procedures

 

We are excited you will be joining us on June 4th, 5th and 6th for the Siege at St. Francis Soccer Tournament.  For a quick check-in, please mail the below items to our attention no later than Friday, May 21, 2010.  You can access most of the forms requested below on our website at http://www.stfrancissoccer.com/main/Tournament/Siege2010.  We will not be returning the listed documents below.  Make a copy for your records before sending the originals to our attention:

 

            1.             A copy of your team’s Valid State Roster

                2.             Original Tournament Roster / Waiver of Liability – form on SFSC website

                3.             Original Guest Player Form – form on SFSC website

         4.             Original Permit to Travel Form (out of town teams only) – visit the Indiana Youth Soccer website www.indianayouthsoccer.org/service/travel_permits.aspx for more information on travel permits. 

 

Mail items #1 – 4 above to Angela Woodlee, c/o Siege at St. Francis 2010, P.O. Box 17151, Indianapolis, Indiana 46217 no later than Friday, May 21, 2010.

 

The following items need to be present when teams check in at the tournament:

 

                1.             Player Passes (Birth Certificates are not acceptable.)

                2.             Medical Release Forms (form available for use on SFSC website)

                3.             Pre-order T-shirts Form (if applicable)

  

Teams not properly checked in prior to their first game on Saturday may forfeit their first game.  

 

OPEN REGISTRATION will occur on Thursday, June 3rd from 6:00 – 8:00 pm and Friday June 4th from 6:00 pm to 9:00 pm at St. Francis Soccer fields located off Arlington (there are directions to the fields on our website).  SATURDAY REGISTRATION IS BY APPOINTMENT ONLY – call Angela Woodlee to reserve time for check-in.  Contact number is listed below. 

 

If you have other scheduling requests due to regular league games or personal schedules, please let us know by our team cut-off deadline of May 20th.
